Chronic Disease Prevention & Management Nurse

About the role

The Chronic Disease Prevention & Management Nurse (CDPM) provides expert self‑management support for individuals living with chronic conditions such as diabetes, COPD, hypertension and coronary artery disease. Working within Belleville & Quinte West Community Health Centre’s primary‑care team, the nurse delivers health education and promotion through individual and group formats and helps shape related programs.

Key responsibilities

• Perform assessments, triage, health education/counselling, history taking and other clinical activities in line with College of Nurses of Ontario standards and evidence‑informed guidelines.
• Provide high‑quality client care independently using medical directives and collaboratively within an interdisciplinary team.
• Collaborate with Nurse Practitioners and physicians to develop, revise and implement evidence‑informed medical directives.
• Deliver care in a trauma‑informed manner that respects client self‑determination and acknowledges social determinants of health.
• Provide back‑up coverage in the general clinic nursing role as required.
• Participate in the development, implementation, monitoring and evaluation of treatment, education, counselling and health‑promotion programs for individuals with or at risk of chronic conditions.

Required profile

• Registered Nurse meeting College of Nurses of Ontario standards of practice.
• Ability to work independently with medical directives and as part of an interdisciplinary team.
• Commitment to trauma‑informed, client‑centred care and awareness of social determinants of health.
