Team Lead – Nursing (RN/RPN) – Homeless Health Program

About the role

The Homeless Health Program (HHP) at WellFort Community Health Services seeks a dynamic nursing professional to lead both in‑shelter and outreach teams. This temporary eight‑week, full‑time position provides clinical leadership, operational oversight, and mentorship to ensure high‑quality care for vulnerable populations.

Key responsibilities

• Supervise interprofessional HHP teams, coordinating in‑shelter and community outreach activities.
• Develop and review draft staff schedules in line with employment standards and WellFort policies.
• Conduct regular team meetings and huddles to communicate key performance indicators and service updates.
• Provide clinical direction, ensuring patient care aligns with professional nursing standards and organizational procedures.
• Collaborate with the primary care team to contribute to client care plans and delegated acts.
• Partner with external service providers to facilitate integrated care pathways.
• Document identified problems using the prescribed framework and implement evidence‑based interventions.

Required profile

• Registered Nurse (RN) or Registered Practical Nurse (RPN) with current licensure in Ontario.
• Demonstrated ability to act as a professional role model, mentor, and change agent.
• Strong knowledge of employment standards, WellFort policies, and clinical nursing standards.
• Experience supervising interprofessional teams in community or shelter settings.
• Excellent communication and collaboration skills with internal and external partners.
