About the role
The South Riverdale Community Health Centre is seeking a Nurse Practitioner to join its primary care team. This role combines independent clinical practice with collaborative teamwork to deliver comprehensive primary health services to the Centre’s diverse client base.
Key responsibilities
- Provide primary health care to clients of the Centre, focusing on health promotion, education, and disease prevention.
- Function both independently and as an integral member of the multidisciplinary primary care team.
- Engage with clients and the community to ensure equitable access to high‑quality health services.
Required profile
- Registered Nurse Practitioner with a focus on primary health care.
- Commitment to working onsite at the Centre’s Toronto location.
- Alignment with the Centre’s mission to improve physical, mental, spiritual, and social well‑being for people facing barriers.
