Primary Care Nurse Practitioner

About the role

The Primary Care Nurse Practitioner joins the interdisciplinary Primary Care Team at ConnectWell Community Health, delivering comprehensive primary health services to residents of Renfrew County. The role emphasizes health promotion, client education, and support for self‑management in both clinic and community settings.

Key responsibilities

• Provide primary health care assessments, diagnoses, and treatment plans to clients.
• Deliver health‑promotion and education initiatives to encourage self‑management.
• Collaborate with physicians, nurses, and allied health professionals as part of the Primary Care Team.
• Adhere to the College of Nurses of Ontario Standards of Practice.
• Participate in on‑call telephone service rotation and work occasional evening hours.
• Travel within the service area using a personal vehicle as needed.

Required profile

• Completion of a recognized Primary Care Nurse Practitioner program.
• Master’s degree in Nursing or equivalent education and experience.
• Current, unrestricted RN (EC) registration with the College of Nurses of Ontario.
• Minimum two years experience as a Nurse Practitioner in a primary‑care or community setting (preferred).
• Demonstrated ability to work effectively in an interdisciplinary team and provide leadership in professional practice.
• Valid driver’s licence and access to a vehicle.
